Yet in practice this remains difficult: barriers are too high, the reporting process is unclear, or employees simply do not know what to do when they see something unsafe. The result is that dangerous situations go unnoticed for too long.

In this guide you will find a step-by-step approach to setting up a working system that enables employees to report safety risks themselves, directly from the workplace. From establishing an accessible reporting channel to measuring participation: after working through these steps, you will have a concrete approach that works for operational teams.

Start by creating an accessible reporting channel

The reporting channel is the foundation of your entire approach. If employees have to take extra steps to submit a report, they will not do it. Choose a channel that fits the way your employees already communicate, without requiring them to create a new account, download an app, or remember a password.

Identify which communication tools employees already use on a daily basis. Think about their phone and, specifically, apps they already know from personal use.

Choose a channel that creates no barrier: no mandatory login, no complicated interface, no dependency on a work email address.

Test the channel yourself: can an employee submit a report in under a minute without any prior explanation?

Make sure the channel is available at the workplace itself, even when there is no computer nearby.

After setting it up, check whether the channel works for all employees in your team, including colleagues who speak other languages and those who are less digitally confident. If someone needs help to submit their first report, the barrier is still too high.

Define clear reporting categories and formats

An open prompt such as “report your safety risk here” does not work. Employees will not know what is expected of them, and the reports that come in will be difficult to compare or follow up on. By defining categories and a format in advance, you make reporting concrete and processing more efficient.

Determine the categories that are relevant to your work environment. Consider: hazardous situations, near misses, defective equipment, unsafe behaviour, or missing protective equipment.

Create a short, structured format for each category. Use fixed questions such as: What did you see? Where did this happen? When did this happen? Is anyone involved?

Keep the format brief: three to five questions is sufficient. More questions means fewer reports.

Make sure employees can also report anonymously where needed, so that the barrier for sensitive situations is lower.

After the first week, check whether the reports coming in are usable. If they are too vague, your questions are not specific enough. If they are too long, you are asking too much. Adjust the format based on what you receive.

Train employees to recognise and report risks

Employees will only report when they know what a safety risk is and understand why reporting matters. Without that foundation, many situations go unreported — not because employees are unwilling, but because they do not recognise something as worth reporting.

Use short, practice-oriented instructions to teach employees what to look out for. Link those instructions to concrete examples from their own work environment: a loose floor mat in a warehouse, a missing safety sign on a construction site, a machine making unusual noises.

Create a short instruction that explains what a safety risk is, with three to five recognisable examples from the relevant sector. Using a toolbox meeting as a safety instruction is an effective approach for this.

Explain what happens with a report: who receives it, what is done with it, and how quickly does the organisation respond?

Repeat the instruction regularly, so that reporting becomes a habit rather than something employees hear once at the start.

Make sure the instruction is available in the language employees themselves speak, so that colleagues who speak other languages can also participate fully.

After the training, you know employees are ready when they can describe without hesitation what they would report and how. Feel free to check this with a short knowledge check: it gives you insight into who has understood the instruction and who may need additional attention.

Activate a feedback loop after every report

Employees stop reporting if they never hear what has been done with their report. A feedback loop is therefore not a nice-to-have: it is what determines whether your reporting system continues to work in the long term.

The feedback loop does not need to be complicated. The key is that the employee who submitted a report knows that their report has been received and is being taken seriously.

Send a confirmation immediately upon receipt: “We have received your report and are looking into it.”

Follow up within a reasonable timeframe on what has been done: has the risk been resolved, has an investigation been started, or has another action been taken?

Share a periodic summary of reports and actions with the wider team. This shows that reporting has an effect and encourages others to do the same.

Thank employees who report. Recognition reinforces the desired behaviour.

After a month, check whether employees who reported earlier have submitted new reports. If that is not the case, the feedback loop is probably not strong enough. Ask directly what they heard after their last report.

Measure participation and improve with reporting data

With a reporting channel, clear categories, training, and a feedback loop in place, you have set up a working system. The next step is to monitor whether the system is actually being used and where there is still room for improvement.

Reports give you insight into who is reporting, how often, across which categories, and whether there are patterns in the risks being reported. Without that data, you are working on instinct.

Decide which figures you want to track: the number of reports per period, the distribution across categories, the average turnaround time from report to action, and participation per team or location.

Set a reporting frequency that suits your organisation, so that you can act promptly if participation declines.

Analyse the categories: which risks are reported most often? Are there locations or teams that never report? These are signals to act on.

Use the data to refine your training and reporting categories. A reporting system only improves if you feed the insights back into the process.

A well-functioning system shows you where the risks are before something goes wrong. Take action if you notice that certain teams are falling behind in participation, and investigate whether the barrier is higher for them than for others.

How we help with reporting safety risks in the workplace

The steps above can be carried out entirely on your own. That said, we know that putting them into practice takes time and attention, especially when working with dispersed teams, employees who speak other languages, or employees without a work email address or laptop.

Our platform makes reporting safety risks accessible via WhatsApp, without employees needing to download an app or create an account. Here is what you can concretely do with it:

Send short safety instructions and reporting formats directly to employees’ phones, in their own language thanks to automatic translations into more than 50 languages.

Record reports and participation in a clear dashboard, so you always know who has responded and who has not.

Build a reporting module in around 10 to 15 minutes, which employees can complete in a few minutes.

Track participation per team, location, or employee and make adjustments where needed.

Would you like to see how this works for your organisation? Schedule a free demo via our calendar and we will show you how to set up a working reporting system that employees actually use.
